Trinity Health Primary Care is located on the side of the Samaritan Center. The best way to reach our clinic is to turn up Shoemaker St and it is the last entrance on your left before the railroad tracks.

Expanded Medicaid in Michigan, also known as the Healthy Michigan Plan, gives health insurance access to uninsured Michiganders that do not have affordable options. Since April 2014, Michigan has accepted applications for the Healthy Michigan Plan. Trinity Primary Care - Detroit offers assistance and resources to our patients with the application process.
